Die Abkürzung NHL steht für:
- Non-Hodgkin-Lymphom, eine Erkrankung der Lymphknoten.
- National Hockey League, die nordamerikanische Eishockey-Liga.

Non-Hodgkin-Lymphom

Artikel sollte dringend überarbeitet werden: seltene Formen werden erwähnt (kutane Lymphome), die häufigsten Formen fehlen z.B. follikuläres Lymphom, WHO-Klassifikation statt Kiel Klassifikation, Therapie viel zu kurz, Rituximab?? etc. etc. Die Non-Hodgkin-Lymphome sind eine vielgestaltige Untergruppe der malignen Lymphome. Zu ihnen zählen: # das follikuläre Lymphom # das Mantelzelllymphom # die chronisch lymphatische Leukämie (CLL) # das diffus großzellige Lymphom # die Haarzellleukämie # das Plasmozytom # der Morbus Waldenström # maligne kutane Lymphome (Mycosis fungoides und andere) # das MALT - Lymphom ( mucosa associated lymphoid tissue )
Ursache
Die Pathogenese der malignen Non-Hodgkin-Lymphome ist noch weitestgehend ungeklärt. Pathogenetisch können Virusinfektionen ( HTLV-1-Viren beim südjapanischen T-Zell-Lymphom, EBV-Viren beim Burkitt-Lymphom, Kaposisarkomvirus ) und chromosomale Aberrationen ( t[8:14], t[14:18], t[11,14]- Translokationen ) eine Rolle spielen. Meist kommt das Non-Hodgkin-Lyphom in den Teilen Afrikas vor. Die Heilungschancen sind bei Erwachsenen so gut wie kaum da, bei Kindern bestehen da schon mehr Heilungschancen, die sich aber nur durch passendes Knochenmark und anschlagender Chemotherapie wirklich erfolgen können.
Symptome
# meist nicht schmerzhafte Lymphknotenvergrößerung # Leistungsminderung, Müdigkeit # evtl. B-Symptomatik ( Fieber >38°C, Nachtschweiß, Gewichtsverlust ), prognostisch ungünstig Im Labor können sich folgende Veränderungen finden: # Anämie # Leukopenie (Lymphopenie, Eosinophilie) # Thrombopenie # Entzündungszeichen (Blutsenkungsgeschwindigkeit erhöht, alpha2-Globuline und Fibrinogen erhöht) # Eisen erniedrigt, Ferritin erhöht # bei B-Zell-Lymphomen (Plasmozytom, Morbus Waldenström) monoklonale Gammopathie # häufig Antikörpermangelsyndrom
Diagnose
Die Diagnose wird histologisch anhand der Biopsie eines betroffenen Lymphknotens gestellt. Neben der histomorphologischen Beurteilung werden spezielle Färbetechniken angewandt, um das gewonnene Biopsiematerial genau klassifizieren zu können. Die derzeit gültige WHO-Klassifikation hat die früher verwendete Kiel- bzw. REAL-Klassifikationen weitestgehend abgelöst. Für die genaue Stadieneinteilung sind weitere Untersuchungen notwendig: # Röntgen Thorax # Sonographie des Bauchraums # CT von Hals, Thorax und Abdomen # Knochenmarkspunktion zur Gewinnung der Knochenmarkshistologie und Ausschluss eines Knochenmarkbefalls Eine genaue Klassifizierung und Stadieneinteilung ist für eine gezielte Therapie unerlässlich.
Stadieneinteilung
Weltweit ist heute eine Klassifikation der bösartigen Lymphome üblich, die 1971 in Ann Arbor, Michigan entwickelt wurde.
- Stadium I: Befall einer einzigen Lymphknotenregion
- Stadium II: Befall von 2 oder mehr Lymphknotenregionen
- Stadium III: Befall auf beiden Seiten des Zwerchfells
- Stadium III 1: subphrenische Lokalisation, beschränkt auf Milz,
- Stadium IV: Disseminierter Befall oder Befall von Leber oder Knochenmark Zusatz A = keine Allgemeinsymptome (Fieber, Nachtschweiß, Gewichtsverlust), B = mit Allgemeinsymptomen, E = Befall ausserhalb von Lymphknoten und Milz.
Therapie
Die Therapie richtet sich nach dem Ann-Arbor-Stadium der Erkrankung.

- Wirkungsstätte eines bedeutenden Amerikaners
- wichtige nationale Gedenkstätten
- herausragende Beispiele für Gestaltung und Bau
- charakterisch für eine bestimmte Art zu leben (way of life) oder
- von archäologischem Interesse sind Mehr als die Hälfte der knapp 2.500 NHLs befinden sich in Privatbesitz. Die National Historic Landmark Stewards Association hat sich der Erhaltung und dem Schutz der NHLs verschrieben. National Historic Landmarks sind eine Untermenge der im National Register of Historic Places verzeichneten Stätten.
